Terms of Service: Hess Chiropractic Life Center

Please read these Terms of Service (“Terms”) carefully. By scheduling an appointment, using our services, or visiting our clinic, you agree to be bound by these Terms.
1. Scope of Services
Hess Chiropractic Life Center provides chiropractic care, wellness consultations, and related health services. All services are provided by licensed professionals according to the standards of chiropractic care.
2. Patient Responsibilities
You agree to provide accurate, current, and complete health and personal information.
You understand that chiropractic care may involve hands-on procedures, and you will communicate openly with the provider regarding your comfort, health concerns, or any adverse reactions.
You agree to follow the recommended care plan to achieve the best possible outcomes.
3. Appointments and Cancellations
We kindly request at least 24 hours’ notice for appointment cancellations or rescheduling.
Missed appointments or late cancellations may be subject to a $67 cancellation fee.
Repeated no-shows may result in discharge from the practice.
4. Payment and Insurance
Payment is due at the time of service unless other arrangements are made in advance.
We accept cash, credit cards, debit cards, and HSA. If using insurance, you are responsible for understanding your coverage, deductibles, and copays.
Any balances not covered by insurance are your responsibility and must be paid promptly.
5. Health Information and Privacy
We comply with all applicable federal and state privacy laws, including HIPAA.
Your health records are confidential and will only be shared with your written consent or as required by law.
You have the right to request access to and correction of your health records.
6. Limitations of Care
Chiropractic care is not a substitute for emergency medical treatment.
If you experience a medical emergency, call 911 or go to the nearest emergency room.
We do not guarantee specific results, as individual outcomes may vary.
7. Informed Consent
By receiving services at Hess Chiropractic Life Center, you acknowledge that you have been informed about the nature of chiropractic treatment, potential benefits, risks, and alternatives, and you consent to care.
You have the right to withdraw consent or discontinue care at any time.
8. Children and Minors
A parent or legal guardian must accompany patients under 18 years old.
The parent or guardian is responsible for all decisions regarding the minor’s care and payment.
9. Practice Policies
We reserve the right to update fees, services, and policies with reasonable notice.
We reserve the right to refuse or discontinue care if patient conduct is inappropriate or disruptive.
10. Governing Law
These Terms are governed by the laws of the state of Georgia, without regard to conflict of law principles.
